We prefer to keep a flat fee for simplicity and transparency with families and providers.

All visits are in-person. We do not provide virtual nurse delegation visits.

Nurse Delegation

• $400 flat fee. Initial long-term care comprehensive assessments with a preliminary care plan

• $300 flat fee. Reassessments due to a significant change in condition, or for annual assessments with a care plan

• $200 flat fee. Routine supervisory visits. This includes changes within that time frame (90 days), such as new orders, new tasks, on-site visits, phone calls, paperwork updates, and new caregiver training.

• $250 flat fee if injections are involved. Routine supervisor visits. This also includes changes within that time frame (90 days), such as new orders, new tasks, phone calls, paperwork updates, and new caregiver training.

Please note that pricing for Medicaid patients is determined according to reimbursement rates established by the Department of Social and Health Services (DSHS) and the State’s Medicaid program
